Description

To ensure that patients care received and delivered in a timely

and efficient manner, and is appropriate to the correct care delivery setting

vs. the severity of illness of the patient. 


Collects, analyzes and reports quality measures.

Facilitates staff and physician education related to CORE measures.


Assists Director with coordinating the hospital?s quality initiatives.

Prepares monthly Core measure data using the QRM reporting form.


Demonstrates understanding of all information systems necessary for job performance, including Microsoft excel, word, COMET.


Accurately abstracts clinical information from the medical record for quality data measures.  


Possesses the ability to communicate with discretion and diplomacy with physicians, hospital team members, Department directors and agency surveyors.

Participates on facility committees as requested by Director.

PROBLEM-SOLVING/CHALLENGES: 


Able to comprehend and apply approved criteria to determine the appropriateness of a patients length of stay and diagnostic/consultative services.  Ability to assess and evaluate a patients post hospital medical and non medical needs and to determine the correct resource to meet those needs.  Errors in judgment could result in actions that my inhibit a patient?s recovery, expose patient to potentially harmful conditions/environment, expose self and the hospital to liability and or financial loss.




DECISION-MAKING: 


Demonstrates thorough knowledge and ability to collect appropriate data measurement, analysis and presentation.  Monitors and analyzes data, trends and patterns that identify variances with established practices.  Demonstrates ability to discuss data reports and presentations, identifying trends and highlighting data points that will assist other appropriate personnel in improving performance.  Demonstrates the ability to identify any inconsistencies in data received or errors in data received from other departments or outside sources and corrects data errors.  Assures dissemination of information and outcomes of performance improvement activities.  Collects and analyzes data that assists with evaluation of corrective action plans and reports findings.  Is able to make ?positive? or ?approval? decisions without review or supervision.  Must seek Physician Advisor level review and approval



for ?negative? or ?denial? decisions.



ETHICS AND CODE OF CONDUCT:


 

Incumbent must attend ethics and code of conduct training provided by LRMC Education Department within 30 days of employment and annually thereafter.



COMMITTEE/FUNCTIONAL TEAMS:


Will participate in committees/teams/work groups as assigned.




HIPAA TRAINING:


 

Incumbent must have HIPAA training provided by LRMC Education Department within 30 days of employment and annually thereafter.



WORKING RELATIONSHIPS:  



Demonstrates concern and provides emotional support for patients of all ages and their family members.  Considers age-related factors in the care, assessment, treatment and education of patients.  Applies age-related factors appropriately to neonate, pediatric, adolescent, adult, and geriatric patients.  Has daily contact with physicians and staff members to coordinate patient care and act as a liaison between patient/physician.  Frequent contact with ancillary departments to accomplish goals and objectives. 




WORKING CONDITIONS:


PHYSICAL – Must be able to tolerate long term sedentary activity, also have ability to carry up to 15 lbs, walk moderate distances.  Able to sit and stand for extended times.  Able to walk moderate distances and climb stairs.




MENTAL/VISUAL – Must be able to tolerate long term sedentary activity, also have ability to carry up to 15 lbs, walk moderate distances.  Able to sit and stand for extended times.  Able to walk moderate distances and climb stairs.


ENVIRONMENTAL Normal office environment.  Brief exposure to outside weather conditions.  Possible exposure to contagious diseases.




EQUIPMENT Personal computer, telephone, fax, calculator, and copier.


Qualifications

MINIMUM QUALIFICATIONS:


·         Registered nurse in the state of West Virginia.


·         Minimum four (4) years hospital/clinical experience


·         Working knowledge of approved criteria used for


       assessment/decision making (Interqual)


·         Working knowledge of Core Measure Criteria


·         Willingness and ability to teach others
