Job Summary

The Program/Service Coordinator is responsible for coordinating the flow of patients and progress of the program/service. The Program/Service Coordinator is responsible for all data collection, analysis and reporting needs for the department related to quality and productivity, including, but not limited to descriptive, outcome, structure, and process metrics. This position manages the data repository for the department and advocates for quality, safety, consistency and accuracy in all activities. The Program/Service Coordinator will participate as part of the intra/multi disciplinary team to develop goals for the program/service requirements and/or growth as required by program oversight and/or Monument Health System strategic goals. This position will assist with patient education and identified clinical interventions as identified by licensure scope. This position interfaces with various roles and positions throughout the organization and externally.

Job Description

Essential Functions:


  • Collaborates in the development and implementation of efficient systems to support patient safety, improve patient satisfaction, and enhance provider/program productivity.
  • Acts as a liaison between physicians, hospital clinics, nursing personnel, residents, patients etc. to optimize patient education, care and scheduling.
  • Assists with patient education and interventions for identified patients in accordance with policy, procedure, scope and licensure.
  • Demonstrates effective fiscal management through cost containment measures while ensuring high quality, cost effective care is being provided to patients.
  • Collaborates with medical staff, hospital/clinic personnel and other members of the interdisciplinary team to promote quality outcomes, patient safety, challenge, innovation and/or growth of services.
  • Participates in departmental and organizational committees/teams to assist in achieving identified goals as required.
  • Actively supports, implements, maintains, educates and promotes organizational quality initiatives and goals. Communicates and educates providers and support staff as needed to ensure organizational understanding and compliance with quality/safety standards.
  • Maintain ongoing knowledge of associated legislative, regulatory and/or compliance issues impacting practice and ensures up to date practices.
  • Maintain applicable role related program/licensure certification(s).
  • Establish and maintain applicable program/service related competencies, policies, procedures, etc. as identified.
  • Serves as the program/department’s point person to obtain necessary information to actively facilitate and coordinate appropriate program/patient flow through the continuum of care and processes, both intra and interdepartmentally.
  • Coordination of data; tracking quantitative and qualitative data. Completes relevant data collection, entry and analysis. Create, organize, submit, and maintain all pertinent data to support the department data tracking and/or program compliance/requirements.
  • Responsible for ad hoc data collection, analysis and reporting of both clinical and financial information. 
  • Supports department leaders with monthly financial reporting and trending, as well as the Medical Director with clinical data analysis to determine opportunities for improvement as assigned.
  • Consistently serves as a resource person for department staff, ancillary personnel, and community.
  • Coordinates and participates in processes targeted at the promotion of community health and patient education. 
  • Performs all duties autonomously, requiring minimal guidance in accordance with leadership and department expectations Seeks guidance and direction, as necessary, for performance of duties.
  • Specific job duties may vary based on program/service, facility and/or healthcare setting.
  • All other duties as assigned.

Additional Requirements


Required:
Education – Completion of a nursing education program that is approved by a board of nursing
Certifications – Basic Life Support (BLS) Certification – American Heart Association (AHA) within 60 days of hire and Registered Nurse (RN) – South Dakota Board of Nursing 

Preferred:
Experience – 5+ years of Registered Nurse Experience
Education – Bachelors degree in Nursing

Pulmonary Operations Coordinator
Required:
Education – Completion of a nursing education program that is approved by a board of nursing or an associates degree in a health related field and
Certifications – Basic Life Support (BLS) Certification – American Heart Association (AHA) within 60 days of hire and
Registered Nurse (RN) – South Dakota Board of Nursing or Respiratory Care Practitioner (RCP) – South Dakota Board of Medical and Osteopathic Examiners, Registered Respiratory Therapist (RRT) – National Board for Respiratory Care (NBRC) or Respiratory Care Practitioner (RCP) – South Dakota Board of Medical and Osteopathic Examiners; Certified Respiratory Therapist (CRT) – National Board for Respiratory Care (NBRC)


Preferred:
Experience – 5+ years of Registered Nurse Experience or Respiratory Therapist
Education – Bachelors degree in Nursing or Bachelors in Health Sciences
 

Physical Requirements:
Medium work – Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
