Department: Inpatient Hospice
Shift/Schedule: Full Time (36 hrs/wk), Night Shift


GENERAL SUMMARY

Works under the supervision of the Nurse Manager of Hospice Inpatient . The RN’s primary job function is to identify patient/family needs and provide supportive care in accordance with the attending physician’s order, the Interdisciplinary Plan of Care, and the Hospice concept and philosophy of care by rendering professional skilled nursing care in the facility. Performs other duties as assigned.


QUALIFICATIONS

Education:
• High School Diploma or successful completion of an equivalent High School Exam Required
• Graduate from an accredited School of Nursing with the minimum of an Associate’s Degree required
• Knowledge of nursing theory and practice required

Licensure:
• RN(OH) required
• BLS within 90 days of hire required


Experience:
• Approximately two years as a Registered Nurse with experience working with terminally ill patients preferred.

JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS



The following is a summary of the major job duties of this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time.
1. Applies nursing principles and techniques in the treatment of patients. Provides nursing care, emotional support and education to the patient/family/caregiver.
2. Communicates and collaborates with physicians and other team members in developing and revising the Plan of Care. Is responsible for the implementation and continued development of the patient care plan and physician’s plan of treatment.
3. Is responsible for the case management of the patient, including supervision of all Nursing Assistants, Unit Clerks and Volunteers.
4. Completes nursing assessment of patients and documents appropriately on patient record.
5. Attends and participates in daily report, team meetings, briefings, in-services, department and staff meetings and other related activities.
6. Understands and adheres to the Facility’s Mission and policies and procedures, and advocates its philosophy.
7. Provides appropriate support at the time of death and through the period of bereavement as determined appropriate.
8. Initiates and participates in discharge planning process as indicated by policy and procedures.
9. Adheres to all policies, procedures and regulations regarding patient care, conduct, safety, fire, security and risk management.
10. Pre-cleans, decontaminates and transports contaminated instruments according to policy
11. Performs intermediate-level disinfection and/ or high level disinfection per the Instructions for Use (IFU) by the manufacture. This includes ensuring the scopes or probes used within the department are cleaned, disinfected, maintained and stored per the IFU’s
12. Performs other duties as assigned.
