Job Summary: The Patient Navigator-Registered Nurse (RN)-Cancer is a skilled oncology nurse and specializes in caring for patients eighteen years of age or older with suspected or confirmed malignancies or a hematologic disorder in the outpatient Cancer Center. Utilizes discretion and independent judgment in applying the nursing process to the provision of age-specific and developmentally appropriate quality patient care.
Case manages and coordinates the continuum of care from referral through the treatment process in collaboration with the physician and the multidisciplinary team. Proactively manages daily clinic activities to ensure efficiency and timely patient visits.
Implements research protocols in collaboration with the research team. Facilitates the empowerment of the nursing profession and embraces new knowledge, innovations and improvements in practice to enhance the patient experience and clinical outcomes.
Performs other duties as needed.
Education and Experience: Bachelor of Science Degree in Nursing required (in progress acceptable). Minimum one year of nursing experience required.
Minimum one year of oncology nursing preferred. One year case management or transitional care management experience preferred.
Knowledge and Skills: Demonstrates proficient competencies in clinical oncology/hematology nursing.
Ability to apply theoretical concepts to clinical practice and continually expands knowledge base and clinical skills with a focus in oncology/hematology treatment modalities. Basic knowledge of healthcare regulations and accreditation required.

Able to: speak, read and write legibly in English (and Spanish preferred) with professional quality; use computer, printer, and software programs necessary to the position, e.g., Word, Excel, Outlook, PowerPoint, electronic medical record, electronic event reporting program; troubleshoot and calibrate patient care equipment; perform technical patient care activities; relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction; think critically; work independently with minimal supervision; perform basic math and statistical functions; manage multiple assignments effectively; organize and prioritize workload; work well under pressure; problem solve; recall information with accuracy; pay close attention to detail; distinguish colors and smells as necessary for patient care; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place; see adequately to read computer screens, medical records, and written documents necessary to position; discern temperature variances through touch.
Licensures and Certifications: Current California Registered Nurse (RN) license required.
Basic Life Support certification from American Heart Association required. Oncology Certification Nurse (OCN) preferred.
