Responsibilities


The CW must possess either a Bachelor’s degree in Nursing (BSN), Social Work, Counseling, Psychology, Marriage and Family Counseling, Human Services, OR a license as a vocational nurse (LVN). Our ideal candidate will have a minimum of 3 years plus of Case Worker/Management experience within a healthcare setting.

The Case Worker (CW) blends the mission of AHF with the Ryan White Medical Case Management (MCC) program for case management follow-up, contact and detail.

Utilizing a strong experience in care coordination and social work, the Case Worker has an understanding the social deterrents of this population, the challenges and barriers to being able to follow through on a plan of care. Reaching and talking to their patients, the Case Worker draws upon their experience with a caring, compassionate and knowledgeable focus.

Combined with a thorough understanding of the Ryan White Program in Los Angeles County, the Case Worker collaborates with the MCC team, Healthcare Center Staff and Providers so that every Ryan White patient receives the support they require and access to available resources. This will include providing education and assistance so that eligibility for the program is understood.

Working directly within the Downtown Healthcare Center in Los Angeles County, the Case Worker reviews charts and progress notes so that contact with the patient is meaningful and maximum assistance is available. Establishing routine contact with patients based on their needs, gives the Case Worker the unique ability to know their patients so that they can work to remove barriers to carrying out their Service/Action/Care Plan goals.



By utilizing your case management experience, you will meet our patients where they are, identify strengths and opportunities for growth and assist them reach their goals.

With your skills of collaboration and engagement, you will bring the essence of the Ryan White Care Act – the safety net for the uninsured HIV/AIDS patients – to our most vulnerable population. Your contacts with patients directly contributes to AHF’s success in meeting the deliverables of our Ryan White contract.

Beyond the scope of the contract, your work contributes measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.
