Sturdy Health has exclusively engaged Avoca Search to recruit an accomplished leader to serve as the Director of Case Management. Located in Attleboro, Massachusetts, Sturdy Health is a fully integrated, not-for-profit, community-driven health system that offers hospital-based care, emergency and urgent care, primary and specialty care with over 25 locations throughout the region.
The Director of Case Management will be an integral leader providing administrative and operational support to the Department of Case Management with oversight for patient throughput, discharge planning, utilization management, and integrated clinical service in accordance with Federal/State legal and regulatory requirements, as well as private payer guidelines. The Director is responsible for maximizing access to and efficient utilization of resources within an integrated multidisciplinary patient care program. In addition, the Director will serve as Co-Chair of the quarterly Utilization Review Committee, collecting, analyzing, and reporting data on key metrics. The Director will ensure effective, timely communication and collaboration with key stakeholders across the care continuum including patients, families, physicians, payors, and representatives of other healthcare entities.
Reporting to the Senior Vice President of Clinical Operations, the Director will drive benchmarks and desired outcomes regarding patient length of stay and readmissions by effectively utilizing established medical necessity criteria for inpatient admissions, observation placement, and transitions of care. The Director will serve as an active leader in daily interdisciplinary patient rounds, observation status and length of stay meetings to facilitate timely, appropriate, and cost-effective patient transitions. S/he will also be responsible for the team of RN Case Managers, Licensed Social Workers, Utilization Review RNs, ED based RN Case Managers and Discharge Planning Assistants to drive efficiency and effectiveness across the care continuum while facilitating seamless care transitions.
The ideal candidate will have a proven track record of three to five years’ leadership experience over Case Management in an acute-care setting. A demonstrated history of effective team leadership while maximizing efficiency and effectiveness of patient care admissions, observation placement and care transitions within a high-risk patient population is preferred. A Bachelor of Science in nursing or a Bachelor of social work degree is required as is a CCM, CMC or CMAC certification. An advanced degree in business, health care administration, nursing, social work, or related health care field is highly preferred.
