REPORTS TO: Clinical Manager
POSITION SUMMARY: Provides personal care services for patients as necessary to maintain their personal comfort. Cares provided are assigned and supervised under the direction of an RN. This
ESSENTIAL DUTIES
- Practices infection control techniques.
- Knowledge and application of policies and procedures of SSRCH.
- Organizes and maintains safe patient care and work areas.
- Documents patient care on a timely basis according to policies.
- Maintains confidentiality in all aspects of the job.
- Responds to emergency situations.
PATIENT CARE
- Provides care and services according to the patient’s plan of care.
- Assists the patient in activities of daily living including personal grooming, feeding, drinking, toileting, bathing, skin care, nail care, dressing, transferring and walking; provides fresh water and nourishment between meals.
- Assists with ambulation, transfer of, and exercise of patient.
- Maintains a clean, safe and comfortable environment.
- Gives emotional support to the patient/family.
- Provides prescribed treatments, after training by RN and a return demonstration.
- Reports status/observations of assigned patients to RN caring for the patient.
- Attends Interdisciplinary Team meetings as requested.
- Assists the nurse in care and treatment measures including giving enemas, changing dressings, collecting specimens, applying compresses, applying other treatments and taking vital signs.
- Assists the nurse in caring for patients with special equipment or apparatuses or requiring special procedures.
- Light housekeeping duties as assigned.
- Promptly answers patients’ call lights and/or requests.
- Works effectively as a member of the Interdisciplinary Team in the delivery of care and assists in the orientation of new employees.
- Performs other related duties and responsibilities as deemed necessary.
HOME CARE
- Assists patient with self-administration of pre-poured oral medications.
- Provides temporary respite care for patient/family.
- Prepares meals and assists patients with eating.
- Promotes patient’s independence.
PROFESSIONAL DEVELOPMENT
- Completes 12 continuous education hours per year.
- Participates in SSRCH-provided education and employee meetings.
- Obtain CHPNA certification within 3 years of being eligible to take the exam.
PERSONAL QUALIFICATIONS
- Affirms the knowledge, philosophy and principles of hospice care.
- Maintains patient confidentiality.
- Instills confidence in the patient by maintaining a neat and attractive appearance and performing duties in a competent and caring manner.
- Required to work reasonable amounts of overtime (when approved by supervisor) and flexible hours. Available to work weekend and holiday rotation schedule.
- Able to maintain healthy boundaries.
WORKING CONDITIONS
- Physical Requirements: Frequent ambulation; constant bending, lifting, twisting, pushing and pulling; push wheelchair with patient, reposition patients, bilateral manual dexterity; climb up and down stairs; able to sit for extended periods of time; manual dexterity to operate computer and other office equipment.
- Home Care: Locate and drive to and from patients’ homes and to meetings at other locations as necessary.
Mental Requirements: Read, speak, write and understand English; cognitive processes to interpret patient condition and contact Team Leader and/or case manager; ability to add, subtract, multiply, divide and ability to work independently with indirect supervision. - Working/Environmental Conditions: Fast-paced changing environment, with multiple directives from several disciplines.
- Home Care: Requires work in patient homes, nursing homes, CBRFs, etc, with no control over environmental conditions.
- Machines, Equipment, Tools Used: Hoyer lift; bed scale; heat lamp; suction equipment; Foley catheter; TED stockings; calculator; thermometer; blood pressure equipment; bed boards; bed cradle; feeding tube; automobile; pager; telephone and computer.
- Exposures: Reasonably anticipated exposure to blood, other body fluids or other potentially infectious materials, hazardous equipment, assorted chemicals, gases/vapors and other pollutants. Possible exposure to patient’s pets and communicable diseases or infections.
- Personal Protective Equipment Required: Assorted gloves; assorted masks, utility gloves; protective eyewear; impervious gowns; impervious aprons.
PROFESSIONAL QUALIFICATIONS
- Certificate/License Requirements: Current inclusion on the State of Wisconsin registry as eligible to provide services in a licensed and certified hospice/home health organization; re-certification as required. Certified in CPR BLS according to the American Heart Association and/or Red Cross.
- Home Care: Valid Wisconsin driver license upon hire and proof of car insurance; renewal as necessary.
- Experience Requirements: Must be at least eighteen (18) years of age. Six months’ experience working as a nursing assistant preferred. Interest in working with terminally ill individuals.
- Education Requirement: Successful completion of a formal Certified Nursing Assistant program; of Hospice orientation program provided by SSRCH; must meet the annual in-service requirements.
- Communicative Skills: Able to communicate directions to patients and family members and volunteer personnel; able to describe patient condition to physician, nurses, patients and family members and offer comfort; able to recognize and respond to non‑verbal cues and communication.
