Responsibilities

Inpatient pharmacist position primarily in internal medicine and cardiology at Mayo Clinic Hospital in Rochester, MN. In this role, the pharmacist provides exceptional care to patients on our internal medicine services or undergoing cardiac procedures or receiving care in our cardiology intensive care unit or general cardiology units. Decentral clinical services consist of collaborative practice with multidisciplinary teams, identification of drug-related problems, anticoagulation management and stewardship, pharmacokinetic monitoring, provision and documentation of recommendations through progress notes regarding optimization of drug therapy, multidisciplinary team rounding, admission and discharge medication reconciliation, transition of care planning, and direct patient care.



This pharmacist will be trained in the roles of the inpatient internal medicine and cardiology pharmacy teams as well as in the central pharmacy at our St. Marys hospital campus with shifts floated among trained areas based on staffing needs. Focused practice areas may be expanded with upskilling and further training based on needs and individuals’ interests.

Job Responsibilities include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Precepting of Mayo Clinic pharmacy and other disciplines’ learners is expected.



Involvement in scholarly activities including quality initiatives and multidisciplinary research is encouraged and supported through extensive institutional and departmental resources. Opportunity for academic appointment at hiring and progression through appointment rank through achievement.


Qualifications


Qualifications:



Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program or Bachelor of Science (B.S.) with a minimum of 2 years relevant experience.



Visa sponsorship is not available for this position; also, Mayo Clinic does not participate in the F-1 STEM OPT extension program

Additional Qualifications:


Commitment to excellence, accuracy, attention to detail and teamwork required. Ability to communicate effectively, work collaboratively with others, organizes time well, practice with an agile mindset, solve problems, and work independently with minimal supervision required.

The following are preferred:


  • Previous experience including PGY1 or PGY2 training and/or equivalent experience of 2 years in related hospital practice
  • Evidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations
  • Relevant applicable pharmacy experience working in a progressive and collaborative practice environment
  • Experience and/or interest in teaching and/or precepting
  • Board Certification in Pharmacotherapy Specialty (BCPS) or Cardiology (BCCP)

License or certification:

License to practice Pharmacy in the State of Minnesota or eligibility for licensure. May not independently fulfill responsibilities reserved only to a pharmacist until license is obtained.
