BJC HealthCare System is seeking a Full-Time Medical Social Worker (MSW) at Missouri Baptist Med Ctr (3015 N Ballas Rd., St. Louis, MO 63131).


Position Details:

  • Day Shift Hours: Mon-Fri (8am-4:30pm or 7:30am-4pm) with rotating weekend & holidays
  • Case Management in an acute medical hospital setting

  • Assessments, discharge planning, & addressing psychosocial needs
  • Collaborate with RN CM to deliver medical case management and discharge planning services in an acute hospital setting.


Experience:

  • Medical Social Work/case management experience (preferred)
  • Acute inpatient hospital experience (preferred)

Overview



Missouri Baptist Medical Center, an acute care hospital in St. Louis County, offers a full continuum of medical and surgical services, including heart care, cancer, women and infants, breast health, gastrointestinal, orthopedic, stroke, therapy, wound and pain management services. The hospital has a 24-hour adult emergency department and cares for pediatric patients at a separate emergency department in collaboration with St. Louis Children’s Hospital.

The Care Coordination Department is comprised of RN Case Managers, Social Workers, Compliant Documentation Specialists and skilled administrative staff. Case Management in the Hospital setting is a collaborative practice model including patients, nurses, social workers, physicians, other practitioners, caregivers and the community. The Case Management process encompasses communication and facilitates care along the continuum through effective resource coordination. The goals of Case Management include the achievement of optimal health, access to care and appropriate utilization of resources, balanced with the patient?s right to self-determination.



Preferred Qualifications


Role Purpose

Under direction of a physician, assists patients and their families in their residences or outpatient/inpatient settings to adapt and plan an interdisciplinary approach to treating the patient-family system.Employees working at an Illinois facility are required to have an Illinois state social work license or be able to take and pass the Illinois state social work licensing exam within 90 days of the first day of employment.


Responsibilities

  • Provides direct clinical services to patients and families.
  • Plans and implements social work services on assigned clinical service or unit in collaboration with the healthcare team.
  • Collaborates with community human service agencies to meet the needs of patients, families, and the hospital.
