JOB SUMMARY:

Performs psychiatric evaluations, individual therapy, group therapy and case-management functions to clients in mental health programs associated with Shore Regional Health. Supports System and Nursing Strategic Plans, and Shared Leadership.

ESSENTIAL FUNCTIONS OF THE JOB:


  1. Conducts intake interviews, psychosocial and mental status evaluations for primary psychiatric triage and care.
  2. Conducts substance abuse assessments.
  3. Develops an individualized plan of care for each client in collaboration with the multidisciplinary treatment team.
  4. Participates in treatment team rounding with patients to develop treatment plans with individual patient goals.
  5. Provides individual therapy, group therapy and case management and discharge planning functions.
  6. Develops treatment materials and educational groups for inpatient behavioral health. Monitors clients’ responses to treatment and revises the plan of care.
  7. Collaborates with the Psychiatrist, Manager of Behavioral Health Services, referral sources and community agencies.
  8. Maintains patient confidentiality.
  9. Maintains accurate and timely documentation in client records.
  10. Identifies opportunities for improvement, promotion and provision of services, including marketing.
  11. Maintains positive customer and staff relations.
  12. Offers support and feedback to peers and staff. Applies principles of caring and healing in interactions with patients and families and with colleagues.
  13. Offers consistent and visible support to team members.
  14. Seeks and supports opportunities for professional growth and development.
  15. Strives to achieve excellence of service Responds to Shore Regional Health codes as appropriate.
  16. Responsible for honest behavior in all matters. To the best of the employee’s knowledge and understanding, complies with all Federal and State laws and regulations.
  17. Maintains the privacy and security of all confidential and protected health information. Uses and discloses only that information which is necessary to perform the function of the job.
  18. Provides structure and mentorship for other disciplines in milieu group activities.

Qualifications

Licensed Certified Masters of Social Work or Licensed Clinical Professional Counselor. Certificate to provide clinical supervision preferred. Minimum of three years’ experience in a Behavioral Health setting. Demonstrates excellent verbal and written communication, interpersonal and organizational skills. One year substance abuse or addictions treatment experience preferred.



The employee in this position must be able to demonstrate the knowledge and skills necessary to provide care based on the age of the patients served. He/she must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status, to interpret the appropriate information needed to identify each patient’s requirements relative to his/here age-specific needs, and to provide the necessary care as described in the department’s policies and procedures.
