Nurse Practitioner – Full Time – Telehealth – Ambulatory Care Network/Columbia Campus

As an essential part of our Ambulatory Care Network (ACN), you’ll help lead a telephone triage center operated by a dynamic team of nurses. It’s an initiative focused on improving call management, increasing access to care, and reducing patient wait times. You will be responsible for among other things to help manage the delivery of telehealth nursing services to a large Spanish-speaking patient population; coordinate prescription refills, assess & evaluate care needs of the calls; provide education to patients as well as to your colleagues. Regularly evaluate the operations of the telephone triage center, and identify and address opportunities for improvement.

This is a Monday-Friday, 9 am to 5:30 pm full time day shift position.

Preferred Criteria:

• Telehealth nursing certification preferred
• ED experience preferred

Required Criteria:

• Masters in Nursing degree in Advanced Practice
• Board Certification in area of specialty
• Current NYS FNP and RN licensure
• Current BLS through the AHA
• Bilingual – English and Spanish
• Minimum of two years NP experience
• Strong communication, interpersonal, and triage skills

$147,112-$158,187/Annual
