Summary:
Manages ongoing, effective operation of the regional functions for case management and social services. Oversees resource and utilization management and discharge planning and incorporates linkages with the community and the various levels of care.

Education:
Essential:
* Bachelor’s degree obtained through a formal four-year program. BSN preferred.

Experience:
Essential:
* Five years of management/clinical experience with at least three (3) years relevant case management experience.
* Knowledge of clinical data interface, InterQual, Case Management documentation, secure faxing, Avoidable Day tracking.

Credentials:
* Licensed as a Registered Nurse in the State of Florida or holds a multi-state nursing license; or able to obtain license prior to start is required.
* Certified Case Manager preferred.
