RN Case Manager

As a Home Care RN, you will:
- Deliver exceptional nursing care to clients in their homes, ensuring they receive the highest standard of medical attention and support.
- Collaborate closely with a multidisciplinary team, including therapists, social workers, and other healthcare professionals, to provide comprehensive care tailored to each client’s needs.
- Follow physician’s orders and work under the guidance and supervision of a Clinical Supervisor to ensure all medical protocols and care plans are meticulously followed.
- Build meaningful relationships with clients and their families, offering not only medical care but also emotional support and reassurance during their health journeys.

Responsibilities
- Skilled Nursing Care and Comprehensive Assessment
- Teaching, Supervision, and Counseling Clients and Caregivers
- Interdisciplinary collaboration
- Problem solving and critical thinking skills
- Monitors patient progress to ensure accomplishment of patient goals
- Oasis documentation
- Provides appropriate care for a range of physician-ordered treatments (PleurX Drain, Finger stick Coagulation checks, wound care, wound vacs, IVs, phlebotomy, Trachs, Peg Tube, Ostomies, Telehealth)
Qualifications
Minimum of one (1) years recent experience as a professional nurse in a clinical setting. Home Health experience preferred. Must complete Point of Care Testing training as part of Department Orientation.

* Valid DL With No More Than 3 Moving Violations
Credentials
Essential:
* RN – Registered Nurse
* BLS – Basic Life Saving certification
