Job Summary:
Supports social work staff in meeting administrative and clinical standards and/or requirements. Provides expert psychosocial assessment of and brief targeted intervention for social, emotional and environmental issues that impact client/family ability to optimally benefit from care. Actively collaborates with team to meet client care, outcome management and system improvement goals. Represents the Social Work department on institutional committees/task forces. Provides leadership for department committees/workgroups. Provides clinical supervision and/or education for Social Worker II staff within the department.
A Term position is a staff position that exists for a period longer than six months but is not expected to last beyond one year. Although a position may exist for a specified or indefinite period, staff who accept term positions are not guaranteed employment for the entire period; they are employed for indefinite periods within the limit of the appointment. There are no guarantees of continued employment, either within the term or when the term ends.

The Division of Geriatric Medicine at Vanderbilt is dedicated to promoting healthy aging through patient care, medical education, and research.

Responsibilities:

This position will assist the Division with the GRACE Model and serve as the mental health/Social Worker liaison.

The Term Social Woker will advise and help the if Nurse Practitioner/Social Worker are stuck and need expert advice.

This position will attend the weekly interdisciplinary meeting to discuss patients currently being cared for by the Nurse Practitioner/Social Worker team.

This position will not have any direct patient interactions or responsibilities and will provide clinical advise to the Nurse Practitioner/Social Worker team.
