Shift/Schedule: Contingent (Works as needed)


GENERAL SUMMARY

Works under the supervision of the Manager of Social Services. The SW Student Discharge Planner Assistant’s primary job function is to provide assistance to patients and their families in the form of discharge planning and resource referral appropriate to the ages of the patients served. Performs other duties as assigned.


QUALIFICATIONS


Education:

  • High School Diploma or successful completion of an equivalent High School Exam Required
  • Enrolled in Bachelor’s degree program required


Licensure:

  • BLS within 90 days of hire required


Experience:

  • Participation in a related college practicum preferred

JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS



The following is a summary of the major job duties of this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time.
1. Arranges continuing care post discharge including coordination of services with community resources.
2. Responds to high-risk screenings and physicians’ consults according to criteria.
3. Maintains accurate record keeping in patient’s chart and departmental statistics.
4. Participates in team meetings, non-departmental meetings and on-going hospital committees.
5. Maintains skills through continuing education and current knowledge of community resources.
6. Arranges discharge planning needs by meeting and speaking with patients and families in addition to collaborating with providers, nursing, therapy and other skilled disciplines.
7. Serves as a liaison between patients, families, physicians and advanced care practitioners, organizations, and outside agencies.
8. Performs other duties as assigned.
